Community Support Network
NHS Brooklyn is a member of the Community Support Network, a pre-crisis healthcare referral campaign funded by the Leona M. and Harry B. Helmsley Charitable Trust with oversight provided by The Fund for Public Health in New York City and the NYC Department of Health and Mental Hygiene.
Through online, community-event, and street-level outreach, our staff is raising awareness of and connecting individuals and their families to physical and mental health resources.
We direct those we meet to primary care, insurance, preventive care, and treatment for any medical or mental health conditions before they reach a crisis level. If an individual is found to be in crisis, we will provide a higher level of assistance.

We do not sell any services as members of the Community Support Network, nor do we receive a commission for referrals provided as part of this campaign.
We do not collect name, address, email, and telephone number during our interactions, although we do ask those we meet to assist in providing us with general demographic information and to answer a few simple questions to help us identify the services that will be most immediately helpful.
